รอยแผลเป็นคืออะไร
รอยแผลเป็นเกิดจากการรักษาของร่างกายที่ผลิตโปรตีนชนิดคอลลาเจนมาใช้ในกระบวนการสร้างเนื้อเยื่อที่ถูกทำลายจากการเกิดบาดแผล เช่น ไฟไหม้ น้ำร้อนลวก หรือสิว โดยเฉพาะสิวอักเสบที่มีขนาดใหญ่ ซึ่งคอลลาเจนดังกล่าวมีการผลิตออกมามากเกินไปจึงทำให้เกิดเป็นเนื้อเยื่อที่นูนขึ้นมาจากผิวหนังเดิม กลายเป็นรอยแผลเป็นนูน อย่างไรก็ตาม หากร่างกายผลิตคอลลาเจนไม่เพียงพอ ก็เป็นสาเหตุหนึ่งที่ทำให้เกิดเป็นรอยบุ๋ม โดยเฉพาะบริเวณใบหน้าซึ่งเรียกว่าหลุมสิว ได้เช่นกัน
ชนิดของรอยแผลเป็นนูน

รอยแผลเป็นนูนแบ่งออกเป็น 2 ชนิด ซึ่งสามารถสังเกตได้ดังนี้
- แผลเป็นนูนเกิน (Hypertrophic Scar) คือ แผลเป็นนูน แต่ไม่เกินขอบเขตของแผลเดิม เช่น ขนาดเท่ากับรอยสิวเดิม เป็นต้น
- แผลเป็นคีย์ลอย (Keloid) คือ เเผลเป็นนูนที่ขยายจากขอบเขตเดิมไปมาก
การป้องกันรอยแผลเป็น

การป้องกันการเกิดแผลเป็นเป็นเรื่องสำคัญ โดยเฉพาะการที่มีแผลใหม่ๆ แพทย์จะเริ่มโดยการแนะนำให้ผู้ป่วยนวด หรือการกดบริเวณนั้นๆ โดยทั่วไปแล้วการนวดอย่างสม่ำเสมอในระยะประมาณ 3-6 เดือนแรก เป็นเรื่องสำคัญ และจะช่วยให้แผลเป็นนั้นลดการขยายตัวและนูนเกินได้ ในบางครั้งแผลเป็นที่มีขนาดใหญ่กว้าง เช่นแผลเป็นที่เกิดจากไฟไหม้หรือน้ำร้อนลวก อาจจำเป็นที่จะต้องใช้เครื่องมือพิเศษหรือผ้ารัดหรือ pressure garment โดย pressure garment นี้จะต้องสวมใส่เพื่อที่จะรัดบริเวณที่เกิดแผลเป็น เช่น ใบหน้า ลำตัว และแขน ขา ในช่วงระยะประมาณ 6 เดือนถึง 1 ปีแรกหลังจากได้รับอุบัติเหตุ การนวดก็จะสามารถลดการเกิดแผลเป็นได้เช่นเดียวกัน ดังนั้นในช่วงระยะแรกที่แผลเป็นมีการอักเสบอยู่ การนวดก็จะช่วยลดไม่ให้แผลเป็นมีการขยายใหญ่โตได้
การรักษารอยแผลเป็นนูน

- การใช้แผ่นซิลิโคนปิด แผ่นซิลิโคนนี้จะเป็นแผ่นเจลใสๆที่ทำมาจากซิลิโคน เราสามารถปิดไว้บนบาดแผล หลังจากบาดแผลหายดีแล้วประมาณ 7 วัน การปิดแผลนี้แนะนำให้ปิดตลอด 24 ชั่วโมง เป็นเวลาประมาณ 3 เดือน ซึ่งข้อดีจะทำให้บริเวณผิวหนังที่อยู่ใต้แผ่นซิลิโคนนี้มีความชุ่มชื้นมากขึ้น ทำให้ลดการอักเสบได้
- การใช้แผ่นเทปเหนียว หรือว่า microporous tape ก็จะสามารถทดแทนได้เช่นเดียวกัน แผ่นเทปเหนียวนี้สามารถใช้ปิดลงบนบาดแผลได้โดยตรง และจะทำให้ผิวหนังบริเวณใต้ต่อเทปนี้มีความชุ่มชื้นมากขึ้น ทำให้มีการอักเสบลดน้อยลง
- การฉีดยาด้วยยาสเตียรอยด์ จะลดการอักเสบของการเกิดเป็นแผลเป็นนูนเกินหรือคีลอยด์ได้ ยาที่แนะนำคือ Triamcinolone acetonide ซึ่งเป็นยาฉีดเฉพาะที่ สามารถลดการอักเสบ วิธีการรักษาคือฉีดยาเข้าไปในแผลเป็นโดยตรง แต่ก็อาจทำให้มีอาการเจ็บได้พอสมควรในระหว่างการฉีดยา จะแนะนำให้ฉีดแผลเป็นนี้ในช่วงระยะประมาณไม่เกิน 1 ปีแรกหลังจากที่ได้รับบาดเจ็บ ส่วนใหญ่แล้วจะนัดมาฉีดประมาณเดือนละ 1 ครั้ง ซึ่งความถี่ในการฉีดขึ้นอยู่กับการตอบสนองของยาว่าเป็นอย่างไร
- การผ่าตัด การผ่าตัดมีอยู่หลายวิธีขึ้นอยู่กับรูปแบบของแผลเป็นนั้น ถ้าเป็นกรณีที่เกิดเป็นแผลเป็นนูนเกินหรือคีลอยด์ เราก็อาจจะใช้วิธีตัดออก หรือว่าลดขนาดลงบางส่วน วิธีนี้อาจจะใช้ร่วมกับการรักษาโดยวิธีอื่น เช่น การฉีดยา หรือการปิดด้วยแผ่นซิลิโคนก็ได้ การผ่าตัดมีอยู่หลายวิธี อาจจะใช้วิธีตัดออกโดยตรงแล้วเย็บปิดเป็นเส้นตรง หรืออาจจะตัดออกเป็นรูปซิกแซก เพื่อที่จะให้แผลเป็นที่เกิดขึ้นใหม่มีลักษณะใกล้เคียงกับรอยย่นตามผิวหนัง

ข้อควรระวังในการใช้ผลิตภัณฑ์
- ควรทดสอบการแพ้ก่อนใช้ หากมีผื่น คัน แดง ให้หยุดใช้ และปรึกษาแพทย์หรือเภสัชกร
- กรณีที่มีการใช้กับผิวหน้า ควรเลือกผลิตภัณฑ์ให้เหมาะสม เช่น ผู้มีผิวหน้ามัน ไม่ควรใช้ชนิดออยล์ หรือผู้ที่มีผิวแพ้ง่าย หรือเป็นภูมิแพ้ผิวหนังควรปรึกษาแพทย์ก่อนใช้
